The article raises the question, whether always it is possible to say what the system (or equipment) state is safe from the point of view of the fail-safe design requirements. The paper starts by the analysis of conformity of design solutions of NPP «Fukushima-Daiichi» to the fail-safe design requirements. To reach an informed answer to the question the paper suggests usage of a risk-informed approach. Also examples of the application of this approach are provided.
